Ten Years

For those of you who have been reading this blog forever I write a throwback post to say it's been exactly a decade since the day I found out I was pregnant with Tiny Boy, after taking a home pregnancy test probably way too soon.  If you've been reading that long, you know there was a lot of drama <ahem> that followed. Somehow that little embryo stuck around and turned into a 9-year-old boy child who came home from school talking about some horrible lesson that was "insulting to mothers and video games." (HUH?!)

Anyway, I wouldn't care about St. Patrick's Day but I have that.  And this year I have a martini glass and some Midori with which to celebrate.

The Deep Archives

I've been (slowly) dealing with paper organization and my monstrously overstuffed filing cabinet. This week that has involved taking out large chunks of the cabinet, relocating the files into tote bags from writers' conferences, culling, relabeling, and refiling.

Then I got to the (sizable) chunk dealing with medical records, which I was ostensibly saving for research purposes.  Ob/gyn records from freakish bleeding episodes in grad school, fertility treatment and miscarriage round 1, pregnancy with LG, fertility treatment and miscarriage round 2, pregnancy with Tiny Boy.  Donor sperm.  Donor egg.  All ye old IVF paperwork in folders from Dr. Gorgeous's office.

Could not bring myself to look at any of it.  Just stuffed it back in there.

Who said repression doesn't work as a life choice?

Representations of Fertility Treatment?

So, it appears that I really do want to write about Private Practice, as I've already spewed out several pages.  In the interests of "research"--can I just say I love working on popular culture?!--are there other recent TV shows/films that deal with IVF and infertility (donor egg stuff would be particularly helpful, if it exists)?  I'm more interested in drama than comedy, but I'll happily accept any suggestions that come my way.  Thanks!

p.s. Come to think of it, if there are novels/works of fiction that you know of that deal with these issues, that would be great as well.

Ten Years: A Reproductive Autobiography

A decade ago, on the cusp of 28 and applying for jobs, I decided that I wanted to become an SMC. I was talking on the phone to my ex, referred to here as Dr. Boy, and he was prattling on about jobs that looked good for me, and I remember whining, but I just want to have a baby. A year later, after my first semester on the tenure track, I started hunting down REs in the area and within a couple months was sitting in her office for a consult. You're twenty-nine, she said, and you want to do donor insemination? She wasn't judgmental, and she certainly didn't say no, but it was clear that I was a first for her.

I came home from that appointment with an infertility diagnosis (628.9), which was clearly for insurance purposes alone. Other than a suspected case of luteal phase defect, there didn't seem to be anything to worry about. HSG showed my tubes were open. All my blood work—at least ten vials, I remember I almost passed out—came back fine. I did IUI #1 in June 2002 and started bleeding at 8dpo. My progesterone level came back at a measly 7, but by then it was too late to start supplementation. I did two more unmedicated cycles (badly timed, due to travel) and then turned to Clomid, which is what the RE had recommended all along. I got pregnant on my third Clomid cycle (#6 total) and experienced what felt like the longest miscarriage in human history. Almost six weeks of waiting to bleed naturally, followed by methotrexate and eventually a D&C. I actually started lactating afterward. In all of this, that was probably the lowest point of my life. Happy 30th birthday.

Wonderful Former RE (WFRE) had me take a mandatory 3 months off from ttc attempts. I started trying again in March, Repronex this time, strategically timed not to increase number of follicles but just to boost follicular development. I'd had a miscarriage, I'd been diagnosed with antiphospholipid antibody syndrome (at my request, she did an entire RPL workup, even though I'd only had one loss), and she absolutely did not want me pregnant with twins. I hated Repronex, and even at a measly ½ amp for 3 days, I ended up ovulating around day 10. I asked if I could try Clomid again, and only managed to ovulate even earlier. Then I had a cyst and was told I needed to take a cycle off; I did it anyway, unmedicated, ended up with 2 mature follicles. A little less than nine months later, after a whole lot of heparin and monthly ultrasounds, I gave birth to LG. Lucky Cycle #10.

My plan was to have two children five years apart. So when I hit 35, and LG was 3.5, I had my ob/gyn check my hormone levels. Everything checked out fine (my FSH was maybe 7?), other than a still wickedly low progesterone level. I really wasn't ready to start trying; professionally this was a low-point for me (my third year review) and I have to say the absolute worst age for LG was not 2 but 3.5. We're lucky we both lived through that year. Then she turned 4, I got through my review and had a semester off, and I bought 6 vials of her donor.

I had no reason to think I wouldn't get pregnant relatively easily. I thought maybe the universe owed me (ha!) and I might even get pregnant more quickly than I did with LG. I did two unmedicated cycles and one clomid cycle with Dr. Evil. On my second Femara cycle with Dr. Gorgeous (May 2009), I got pregnant. Very low but appropriately doubling betas; I asked for a fourth because I was paranoid and of course the pregnancy was over by 6 weeks. Repeat RPL testing: MTHFR. Start Folgard and heparin. Antibiotics to rule out infection. Still, the math said I was fine: I could easily write off the three cycles at Dr. Evil's (they didn't check sp*rm count, but the samples at Dr. Gorgeous's had low numbers and one was riddled with bacteria they needed to rewash, likely from being transferred from Dr. Evil's place). Even then, I was pregnant within 5 cycles.

I did another Femara cycle (June), an IVF turned to IUI (July/August), a real IVF (September/October 2009), a Follistim cycle (Nov 2009) that resulted in a freaky bleeding episode at 9dpo that Dr. Gorgeous could only explain by saying it was a very early loss. I plowed ahead and did an unmedicated cycle (Dec 2009); it was around then that she brought up the possibility of donor egg. But I figured that it took me 10 cycles to conceive LG when I was 29-30, so, really, there wasn't a reason to rush things. I wasn't happy about how long things were taking, but it wasn't completely unexpected. She agreed, and said the only reason to do donor egg (at newly 37) was if it was more important to be pregnant now than it was to be pregnant with my own eggs.

I took the winter off. Then I did two Femara cycles (March/April, 2 follicles). May 2010 brought a weird Femara-oops-there's-a-cyst-unmedicated-no-make-that-Follistim combo and a micropolyposis diagnosis (2-3 follicles). More antibiotics. June: Femara/Follistim hybrid (2-3 follicles). July: Menopur cycle from hell (1 follicle). August: IVF attempt #3 turned to IUI (3-4 follicles). September: unmedicated, questionably timed (2 follicles?). October: ½ dose Femara (1 follicle). November: unmedicated, questionably timed (1 follicle). December 2010: simultaneous Femara and Follistim (3 follicles, perfect lining on scope, E2 over 1100, progesterone at 62). And still, nothing more than a faint second line that could have been an evap line at 9dpo.

Diagnoses: ovarian dysfunction, diminished ovarian reserve, poor responder, low AMH, recurrent pregnancy loss, micropolyposis, APA, MTHFR. Dr. Gorgeous said I've been determined, more determined than most of her patients. I've tried DHEA supplementation, acupuncture, royal jelly, and more supplements than I care to name. I take Folgard, Lovenox, baby aspirin, and 400 mg Prometrium to deal with immune issues. She said we've given my body a fair chance. She said if I need to do another IVF we can do it, but she's pretty certain egg quality is an issue (3 beautiful follicles, she said, and still, nothing). When I asked for her opinion, WFRE told me in the spring that yes, DOR seemed likely but too early to move to DE; give it a few more months. Dr. Slideshow said in October that he'd give me a 5-35% chance on IVF with my own eggs, most likely no higher than 17% vs. 75-80% with DE.

I remember when I was 29 and attending a local SMC meeting one of the women who eventually adopted said she got pregnant on her 18th IUI cycle and then miscarried. I couldn't fathom doing 18 cycles. I've now done, I think, 20. There was another woman whose history I didn't entirely understand but had tried for a long time before doing DE around age 48. She was old enough to be my mother, living in an area of the country where women her age were routinely grandmothers. I looked at her and wondered how long could I keep trying, when biologically I had many many years ahead of me before menopause. And here I am now, still probably years away from menopause, with a decent FSH level, a low-normal AFC, and significantly younger than most women who turn to DE.

Logically, 20 cycles seems like more than enough. That I should have had more than a miscarriage and a possible chemical pregnancy or two in all these months. Dr. Gorgeous truly thinks I'm a good candidate for DE, that my problem is egg quality, but we'll use Lovenox/steroids just in case.

I'm trying to look at the facts. Is it worth spending approx. $9K and a couple extra months for a less than 17% chance of having a child who shares my DNA and the chance to prove to Dr. G and myself that I really did have one good egg in the batch? Do I need to have another cancelled retrieval or another batch of poor embryos in the lab in order to be able to move on?

When my OB recommended a c-section, 21 hrs into an induced labor that was going nowhere, I asked my doula--a natural childbirth advocate, former RN, mother of 6—is this reasonable? Her answer was yes. I still have doubts. I still wonder what would have happened if she'd let me labor just a few more hours. I'm still angry that the birth I wanted was stolen from me. But those feelings pale in comparison to the result, having a healthy baby, one I'd fought very hard to have and honestly feared I'd never bring home alive. Dr. Gorgeous has never told me straight out to do DE, but what she said yesterday--we've given your body a fair chance--was what I needed to hear. I will always wonder if a different protocol—EPP, for instance, or using pregnyl instead of ovidrel, or adding Menopur based on daily LH testing, or triggering earlier or later, or using more or less estrace—might have given me a better shot. But if the goal is the baby, isn't it worth the almost-sure thing?

I have enough superstitions about throw-away natural cycles that it's well worth $600 to forge ahead with an unmedicated IUI in January (someone needs to come confiscate my Follistim pen, really), unless I match with a donor, like, next week and can just go on the pill. But I've had two "last" cycles—beautiful cycles, one of which could have gone to retrieval if I'd wanted it to—that didn't pan out. I'm done, folks. I'm exhausted.

I showed LG a videotape last night of my ultrasounds from pregnancy with her. I want that again. I want that now. And I'm not going to let my vanity or hang-ups about my body's failure get in the way. Dr. Gorgeous is right; if I need to do an IVF, I should do it. But a co-cycle has the potential to ruin a very good thing.

This feels a lot like December 2002, when I was thick in grief with the loss of the baby I'm sure was a son. That loss changed me in profound ways but it doesn't hurt on a daily basis the way it did before LG was born. What I'm feeling now is much the same; I lost a baby who would be turning one in a couple weeks, my last proven pregnancy. I think I'm finally, really, grieving that loss, the loss of my fertility and the last pregnancy with my own eggs. And it really sucks. But the possibility of having a baby in my arms as soon as I can? To let LG have the sibling she so desperately wants, hopefully before she turns 8? I think I'd be stupid not to pursue it. The rest of it? Well, that's what therapy is for.

p.s. Valium is very nice indeed. It's a good thing I don't have any more of it or I might be an addict in the making.

An Epiphany

I've been spinning, mentally. The internal monologue goes something like this: I think I should do DE with Dr. Gorgeous because I can do it sooner and because I'm comfortable with her, but maybe I really do need to give Dr. Slideshow a chance so what is it about him and his clinic that I don't like? But you know, maybe I should try IVF with him, like maybe if my antral follicle count goes back up and my day 3 labs come back fine this time, then maybe I should do IVF with him. I mean, really, will I be okay moving on to DE without trying IVF with someone else's protocol? It keeps going, of course, but you get the gist.

And then it occurred to me:

I DO NOT NEED TO DECIDE ANY OF THIS TODAY.

(Aribitrary deadline for making a decision: December 15, which is also my tentative IUI date. Expect a lot of spinning and blogotherapy before then. But not today!)

Plans and Paralysis

I spent too many hours yesterday tinkering with possible book lists for my Odious Spring Class. See, when I was assigned this class (a year ago) I was operating under the assumption that I'd probably be on maternity leave and probably wouldn't have to teach it. I must have been having a particularly bad day when I submitted a course description because no one in their right mind would try to teach what I pitched to undergraduates. Realizing now that there's no way in hell I want to teach that version of Odious Class but there is in fact a way to make it not odious at all but a good deal of fun (why I didn't see this a year ago is beyond me, let's chalk it up to Hormone Head), I emailed the program chair to ask if it's possible for me to change it. He said yes.

Now I have the possibility of reconceiving this class any way I want. And I'm paralyzed. I have multiple book lists, all equally interesting, all about the same level of work for me (mostly texts I know with a couple new ones sprinkled in). So which to do? Which to do? Which to do? What I need to do is set a deadline to submit my book lists (soon, very soon) and just fucking do it and live with whatever choice I've made. Which, let's face it, has always been okay, in the big scheme of things.

It occurred to me that his compulsive re-planning is identical to the endless mind spinning I am doing about ttc. IVF or IUI? IVF with Dr. Gorgeous or Dr. Slideshow? DE here or there? DE sooner or later? This is your reproductive journey, Dr. Gorgeous has said, there is no right or wrong choice.

So why do I feel like there is? And why do I feel like everything in my life is bound up in this decision?

*

I see Dr. Gorgeous on Tuesday. And I want to have a legitimate conversation about how she does donor egg so I can make an educated choice (I know stats matter, but I think the intangibles, like cycling at home, matter too). And I do want to ask about the IVF vs IUI question, though I'm leaning very heavily to doing a hopped up medicated IUI (either hybrid or Follitim alone) in December. See, if she puts me on the pill for ~7 days, not only will we solve the Thanksgiving problem, but we'll also solve the cyst at the beginning of a cycle problem. And I could do an IUI the last week of the semester, which is really good timing as far as those things go. And then, well, then I think I'm out until spring, unless it seems I can swing donor egg sooner.

Decisions are too hard, people. Feel free to weigh in.

Decisions Decisions

So yesterday, sometime between stripping off her Ann Taylor sweater and snapping on the latex, Dr Gorgeous asked me how the consult went. Well, I said, I think I'll need to make an appointment to talk to you about it. It would appear that my opportunity will be this Thursday (?!) so I've got to get my head around somethings pretty darn quick. Where I'm at seems to change by the minute these days. Some things for you to advise me on (while I sit here drinking sanctioned rooibos tea and ignoring the piles of laundry in my house, and the stacks of student papers to grade!):
  1. Dr. Slideshow recommended repeat hysteroscopy if I do donor egg. I think it makes sense to repeat, no matter what my next big step is. I'll have to see what Dr. Gorgeous, and my insurance company, thinks about this.
  2. I'm wondering if it makes sense to do an endometrial function test or test for NK cells or other autoimmune things I haven't been tested for. This isn't coming out of my appt with Dr. Slideshow so much as my sense that there's something more than DOR going on with me. And really, I don't want to give up my own DNA and shell out $30,000 just to have another miscarriage with someone young thing's eggs.
  3. Although I'm still going to acupuncture, I have not made a move in terms of the Chinese herbs plan and I don't think I'm going to. While I'd like to think "holistic" is a better way to go, the fact of the matter is that I don't have the months required to actually make it a legitimate plan and see if it helps me. That plan also assumes that I actually trust my body. And you know what? I don't.
  4. That is, I'm pretty much ready to bail on my body. I hit 38 in a month. That has always been my Donor Egg Deadline. I'm pretty sure I'll be doing donor egg (not sure where yet), but I also think I need to give my body one last big shot. Will that be a highly medicated IUI? IVF with Dr. Gorgeous? IVF with Dr. Slideshow? And when? Winter break would seem to be a good time, don't you think?
  5. I learned something I don't like about myself. Turns out that a donor's looks matter. Dr. Slideshow gave me the link to the extended profiles page, and I have to say I freaked out when I saw pictures of the donors. A. asked me if I could just not look at them. Seriously, do I have that much impulse control?! I really like the fact that I don't know what LG's donor looks like. I've got a baby picture, that's all, and that's how I want it. Turns out that if I wanted a male donor to be a good date, I want a female donor to be a reasonable fascimile of me. Where are the high-IQed, dark haired Eastern European/Jewish egg donors, I ask you?
  6. Since I haven't actually talked to Dr. Gorgeous about how she does the egg donor program, that's one thing I need to know. On the one hand, it seems to make sense to be to go with the bigger clinic for DE; Dr. Slideshow does something like 50 DE cycles per year, Dr. Gorgeous does more like 10. Her stats are also slighly lower (67% vs. 75-80% live birth rate). But there are other issues to consider, like cycling at home is way easier than cycling out of state, particularly with a school-aged child. And there's something to be said for working with the doctor who's seen you through the ups and downs (mostly downs) of 2+ years of treatment.

I keep spinning, and I don't seem to be getting anywhere. Do I take it as a sign that the reproductive counselor hasn't returned my message to schedule an appointment?

Blogotherapy (more thoughts from Dr. Slideshow)

Well, I will definitely NOT be pregnant before my 38th birthday. OPK I took about half an hour ago was positive, or will be by the end of the day. That means no IUI this month (though I might ask Dr. G to run a P4 level on Tuesday to check). I'm too stunned to even start registering disappoinment. The only time I've ever surged on cd 8, I've been hopped up on Clomid/Femara. And given that my lining was 12+ yesterday...clearly my body is ready to go. Not only does this rule out an IUI this month, it really fucks up the whole schedule I have going. And unless I do something (put myself on the pill? do the estrace/prometrium combo?) my next IUI would likely fall Thanksgiving weekend.

Do I take this as a sign that I need to give up on my body and run to donor egg? Do I take this as a sign that I need to give up the "natural" approach, because clearly it's not doing a damn bit of good? I'll admit there's a part of me that thinks this would be the perfect time to take estrace and do a femara/follistim combo.

*

So as I said yesterday, Dr. Slideshow did not provide me with the sense of clarity I was hoping I'd have. I'll admit that the numbers he gave me for IVF were not pretty (17% live birth rate, though he admitted that's arguably dependent on ovarian reserve which is still in question [he wants to redo AMH level and day 3 labs]). But while he was very professional, he did indicate that he thought Dr. Gorgeous's MDL/Ganirelix protocol is very odd and suggested I do a straight antagonist cycle (no EPP) if I choose to do IVF. He also suggested that the fact that I had an E2 level of 2100+ and had only 2 mature eggs at retrieval indicates a problem with how the eggs were retrieved (though it sounded like his method is the same one that Dr. Gorgeous uses).

Do I want to do IVF? No. But I'm not ready to dive into DE either. If I do decide to do IVF, do I do it with him, for something new? For his lab? For his success rates? I'm honestly not sure.

He also told me that he thought IUIs were a complete waste of time/money, that if it hasn't worked by 12-15 cycles, it won't work (but, I think, even at age 29-30, I needed to do 10 IUIs to conceive LG). And, to be sure, I'd rather spend that $600-700 on something else, or save it, but in the whole scheme of how much this baby project has cost me, really, what's the expense of a few more IUIs? Because, really, that's all I plan to do.

Still don't know what to do folks. As always, observations are welcome!

Ultrasounds and Slideshows

I promise to flesh this post out later tonight. I'm just exhausted and need to be with my girl and my mama for a while.

The gist of it:

1. I will probably not be having an IUI this month, as it's cd 7 and ultrasound revealed a lining of 12.3 (!) and a follicle/cyst at 15+ Did I say I'm out of town and the earliest I could actually show up for an IUI is Tuesday?

2. I liked the doctor, whom I will hereafter refer to as Dr. Slideshow, MUCH more than I expected. My consult--the actual talking part--lasted close to two hours. I think if I'd had more questions, and the nurse hadn't paged him to tell him his next patient was there, he would have kept talking.

3. Nothing Dr. Slideshow said has pushed me in either direction. But most of my questions went to IVF not donor egg, so maybe that says something right there. The numbers say donor egg, but when he recommended doing eSET and their out-of-this-world stats go down to 55% I'm less impressed...

4. His office is not The Spa, which is how I thought about Dr. Gorgeous's place when I first met her. There was no bathrobe to wear, no tea in the waiting room, no chinese art. And he's not nearly as pretty :)

I still don't know what to do. Except schedule a follow-up with Dr. Gorgeous.

Can you say deja vu?

Yeah, so this seems to be my IVF attempt from last July/Aug, with only a few minor adjustments.

Depending on my E2 level I will either be having an IUI on Saturday (at Dr. Gorgeous's office) or Sunday (at the IVF clinic). (Yeah, I guess it's good that I have specimens in two locations currently.) What we ended up with: 18, 14.5, 13, 11.5. We'd both really like to get the 14 bigger and the 13 up to the possibly mature 14, but my E2 level didn't rise a whole lot yesterday, so if it's still plateaued or gone down, tonight is trigger night.

I'm too stunned at how fucking abysmal this is to even register something like disappointed. I'm thankful that (a) she was in the office today so this plan didn't involve a three-way game of phone tag and (b) she's willing to do me on Sunday provided the andrologist at the IVF clinic is game.

This was supposed to be my body's last hurrah before donor egg. Yeah, so much for that plan.

Update: Dr. Gorgeous and I have a tentative date for Sunday at 11:00. The only real question is, will she wear those pink shorts?

Update on All Things Feline and Ovarian

I think I spent 75% of my day on the phone and the other 25% in the car. Outgoing calls: Dr. Gorgeous's receptionist, Dr. Gorgeous's ultrasound tech, IVF Kitten's vet (x 2), the animal shelter where we adopted IVF Kitten (x 3), my acupuncturist, A. and M. Incoming calls: Dr. Gorgeous's andrologist, Dr. Gorgeous's ultrasound tech, the animal shelter where we adopted IVF Kitten (X 3), my acupuncturist, A. Have I mentioned I hate the phone? (Conversations with good friends excepting, of course.)

The upshot of all this phone calling and the driving that went along with it: IVF Kitten is currently hospitalized at his vet, as he really hasn't been able to eat or drink and his congestion seems to be getting worse (don't they make snot suckers [aka bulb syringes] for cats as well as human newborns?). Thankfully his lungs are still okay, so this is more annoying and stressful than it probably is life-threatening. Tomorrow, if it is determined that IVF Kitten still needs hospitalization, he will be transfered to a vet who works for the animal shelter so I will not need to pay. The very very very nice director of the animal shelter said she'd also be happy to neuter him (free of charge) when it comes time for that, to try to mitigate some of these costs. So it will be okay, and I don't need to spend another night panicked that I'm going to wake up with a dead kitten in LG's bedroom.

And now for the ovary situation: Dr. Gorgeous is still not willing to just call an IUI an IUI. On the right, the fucking cyst (which shrunk) is now up to a whopping 17, which seems to be preventing from anything else growing on that side. On the left, I have follicles at 15, 13, 12, and 10 and a few more unmeasured that she hasn't ruled out because they're showing some signs of growth. It seems that if I have anything else over 10 tomorrow IVF might be a possibility. I'm not counting on it, and I think that would warrant having a phone consult to go over my odds of conception and multiples for both scenarios. I mean, if I was 30 I wouldn't consider going to IUI with 5 follicles...but we all know my history.

And I find myself irrationally (?) pissed off that my specimen was transported from Dr. Gorgeous's office to IVF clinic without my consent. Now, it's not a problem to get the specimen back should we go the IUI route...but every other time I've had specimens moved I've needed to sign a new consent form, and I was not consulted. So, WTF? My assumption is that Dr. G herself drove the specimen out there yesterday, which I find a little perplexing, particularly since she's the one who told me to have it shipped to her office.

That is, it's been a very long, very stressful day, and if I weren't being such a purist, I'd be drinking vodka around now. Argh.

The Zen Is Gone

I mean, I'm not completely morose or anything but I've certainly lost that graceful zen thing I had going. I'm sure it's partly the cycle and partly the realization that the summer is ending all too quickly. I went into the summer dreading it, and what I found was that I really enjoyed the pace. I was productive. I got my tenure shit turned in. I enjoyed my kid. I read a lot. I watched a fair amount of TV. I recovered, more or less, from the year. And now it's starting up again in three weeks. Between now and then, I have to finish up one syllabus (to be fair, it's at about 80%) and write another one, gather my teaching materials together and finalize my teaching philosophy statement (due Sept 1, for tenure stuff), and scan/copy the secondary sources for one class (which means I need to pick out the secondary sources first).

That would be plenty without the IVF/IUI drama and the New Kitten drama (A. says I am not allowed to call him IVF Kitten but she did say that the name of the town where Dr. Gorgeous's office is located would make a good name, and I concur; unfortunately, LG does not). New Kitten is not healthy, as it turns out, and I've spent way too much time today feeding him baby food with a 3 cc syringe (sometimes being a fertility drug whore is useful, eh?) in the hopes that we can keep him out of the kitty hospital. I think he will be headed there tomorrow, as he really hasn't eaten much of anything since Saturday and he is a 3 lb cat (the vet said to give him another 24 hrs to get the antibiotics into his system). It's stressing me out.

And then at some point today I realized that I will in all likelihood go into yet another school year not pregnant. I really thought that DHEA + No Menopur would do the trick, but this cycle had such a rocky start. So here I am, headed back for monitoring tomorrow and assuming that the cycle will get converted to IUI with 3 follicles at best. Trying to figure out if it's worth trying to see Dr. G in the next two weeks to map out my plan for the fall, which is supposedly a natural IUI only plan, but I find myself thinking what if I take just a little Femara, or 37IU Follistim or... I need to have the plan in place before I need the plan. I know my next big step needs to be donor egg but I'm just not ready to do it. I can't fathom doing it, and I know what I'm hung up on is not genetics (though that's part of it) but accepting my body's failure to produce viable eggs. And the reality that I've been doing treatment for TWO FUCKING YEARS--with nothing to show for it but an MTHFR diagnosis, a micropolyposis diagnosis, one miscarriage, and one possible chemical pregnancy--is getting to me. I'm at the point--the crazy, crazy, point--when I'd like to be pregnant so much I'd prefer yet another miscarriage to this emptiness.

See, I told you I lost the zen.

In Between

I was hoping today would give me a clear cut answer about the IVF vs. IUI possibility for this cycle, but it doesn't seem like it. I'll see what Dr. G says later today, but right now I'm looking at a 13, 11, 10, and 8 on the left and nothing (but that fucking cyst) on the right. My hunch is IUI with 4 follicles, as in my last IVF attempt what I had at stim day 6 is what I ended up with as mature follicles at trigger time. What I'm worried about is the 5 follicle possibility which is too scary for an IUI and really too few (in my opinion) to warrant doing IVF. (If I wasn't paying out of pocket, it would be a different story.)

All things considered, I'm pretty zen. Need to get some good work done this afternoon before I take New Kitten to the vet.

Small Good Things (Stim Day 4)

  1. None of my top three paranoid fears have come true: I am not going to ovulate on cd 6, I am not hyperstimulating, I am not oversuppressed. Instead I have a couple follicles in that 8-10 range, a lining of 10mm (never happens this early in a cycle), and an E2 level of 308. I wouldn't say I'm optimistic, but things are looking up.
  2. It's a good week for LG: she probably passed her swimming test today, she lost a tooth, and she figured out how to tie her shoes!
  3. We adopted a kitten. He is just the sweetest, most calm little guy, and he's virtually identical in markings to our 6-year-old cat. We are currently debating names. Possibilities include: Mallomar, Mellow, Zen, Peace, Harmony, Blockbuster, Perky, Squeaky, or Atilla, which was the name he came with. You can probably guess which are LG's!
  4. Turkish green beans for dinner. Yum!
  5. I decided that my nighttime TV needed to include more laughter. But I'm really particular about comedy. So I'm currently Netflixing Gilmore Girls, which I began watching when I was pregnant with LG. It's perfect for a trying-to-get-knocked-up single mama who goes for wit and not slapstick.

Stim Day 2, In Brief

Headache. Bloated. Nauseous. Collapsed veins. Sleepless nights. Dr. Gorgeous called last night. Let this work. Let this work. Let this work.

Follistimming

Okay, ovaries, now's the time to prove Dr. Gorgeous wrong. Pretty please?

Back to Reality

I didn't make up that thing about 18. Really. I asked for the number. And I saw the tech write down: LO 10, RO 8.

But today? The usual tech got a usual count on me: 10 total (7 left, 3 right). Apparently they "count a little differently over there [at IVF clinic]."

I've come crashing back to the reality of IVF and my body. Fully expecting this thing to get converted to IUI. But I'd still like the opportunity to try...

Updated at 4:08: Although the cyst is smaller (yahoo!), my E2 level went up today (87). Dr. Gorgeous is letting me start the cycle, with the understanding that it might very well end up as an IUI. She's having me ship my specimen to her office, not the IVF clinic, which I don't take as a positive sign. Anyway...I start microdose lupron tonight and Follistim tomorrow, back for monitoring on Saturday. I wonder when the universe might decide to cut me some slack. I could use a big good thing.

On the Agenda

  • Stop on the way to camp/Dr. Gorgeous's office to get a cup of coffee. Did I mention I've been awake since 3:18?
  • Beg whoever will listen to ask Dr. Gorgeous to call me. And give them my list of questions that seems to be growing by the minute, along with my antral follicle count. (Seriously, WTF?)
  • Look into after school programs for LG.
  • Order gametes. Order DHEA. This will take 3 phone calls minimum. I hate phone calls.
  • Get some writing done. Real writing, not just pages toward a novel that I shouldn't be working on. I am not a novelist. Then again, I'm not an RE either.
  • Read something other than novels.
  • Do yoga. And more yoga. Because right now I am freaking the fuck out, vacillating between this cycle will be cancelled before it ever gets off the ground and I am looking at OHSS at max dose Follistim. I *really* need to talk to Dr. Gorgeous.
  • Marvel at those antral follicles. (I'm putting it out here now: I'm guessing there will be 20+ when we take a peek in a few hours.) Who knows how this will turn out, but this is the first thing resembling good news on the fertility front since, oh, 2003.

And now for the update: the cyst is still there (same exact size as it was a week ago) but my E2 level continues to drop (down to 63). One more day of Ganirelix, and then I think she's letting me start the cycle. Although it's not entirely clear to me if that means start as IVF and see what happens or just assume it's a Follistim IUI. But yeah, 18 antral follicles (my highest in a year has been 12) and things are still popping like crazy in there (have I mentioned I've been on Ganirelix for a week already?). I guess that means the DHEA is doing something right because I have no other explanation. This is good, right? This is really good, right?

Have I said that IVF turns me into a raving lunatic? Poor A.

To the cyst on my right ovary

Pop, dammit.

Because, really, having all my plans thwarted by a fluid filled sac smaller than an average grape seems, frankly, ridiculous.

So, pop, dammit.

I'm ready to get on with this cycle. And the rest of my life.

Please.
